The ISCP review cases where a child has died or been seriously harmed and lessons for improvement of multi-agency working could be identified. Any situation where a child has died or been seriously harmed is tragic. We owe it to those children to identify things that could be improved or done differently and possibly prevent similar tragedies in the future.

Lessons from local case reviews
The ISCP conduct case reviews at a lower level than serious case reviews when it is recognised there might be some learning for multi-agency practice. Learning from these cases is disseminated to relevant services through case review action plans and multi-agency training. The ISCP also produce research briefings to highlight learning from local and national reviews.

The triennial analysis of serious case reviews
The latest triennial analysis of serious case reviews: 'Complexity and challenge: a triennial analysis of serious case reviews 2014 to 2017' was published by the UK Government, Department for Education on 4 March 2020. It builds on the learning from previous analyses by the research team from the University of East Anglia and the University of Warwick.
